Adult Day Support

CLW Adult Day Support (ADS) helps individuals build connections within their community. With this service, people can learn how to meet and talk with new people, what activities are in their community, and how to get places independently. While taking part in ADS services, individuals will also receive support with any personal care needs identified through the person center planning process.

Vocational Habilitation

CLW Vocational Habilitation (Voc Hab) provides prevocational services to participants that further habilitation goals such as attendance, attention to task, problem solving, communication skills, and interpersonal skills through person centered planning. Our Voc Hab services offer a variety of paid work opportunities and a vocational based ongoing vocational curriculum.

Nursing Services
Each of our locations have a dedicated nurse (LPN or RN) onsite to oversee the health of the people we serve, to provide medication management, and to deliver staff training. Additionally, we have medication administration certified staff to ensure consistent, reliable care. They are utilized during transportation, community integration, and as needed to continue providing safe, compassionate support at all times.

Community Employment

CLW Community Employment includes job placement and coaching services that support participants in finding and thriving in meaningful employment. Community employment promotes independence, inclusion, and personal growth. We focus on each person’s strengths, interests, and goals to match them with the right employment opportunities.

Community Integration

Community Integration is a way to deliver services through ADS or Voc Hab that purposefully focuses on individual interests.  It is about providing opportunities to access people and places in the same manner as the broader community and making connections based on shared interests. CLW acknowledges that services delivered through community integration looks different for each individual.

Non-Medical Transportation

CLW offers transportation for all of our participants. This service is not only an option for to and from our physical locations, but also to and from community locations to take part in activities chosen by the participants.

Enclave Employment Program

Our Enclave Employment Program provides participants with hands-on job experience in local businesses with support from trained staff. In doing so, participants build job skills, earn income, and gain independence.
