Adult Day Support

CLW Adult Day Support helps individuals build connections in their community. With this service, people can learn how to meet and talk with new people, what activities are in their community, how to get places independently, and what jobs are available in their area. While taking part in Adult Day Support services, individuals will also receive support with any personal care needs that they have expressed through the person center planning process.

Vocational Habilitation

CLW Vocational Habilitation provides prevocational services to participants that further habilitation goals such as attendance, attention to task, problem solving, communication skills, and interpersonal skills through person centered planning.

Community Integration

Community Integration is a way to deliver services through ADS or VocHab that purposefully focuses on individual interests.  It is about providing opportunities to access people and places in the same manner as the broader community and making connections based on shared interests.  CLW acknowledges that services delivered through community integration looks different for each individual.

Non-Medical Transportation

CLW offers transportation for participants not only to and from CLW but also to community locations to take part in activities chosen by the participants.
